2001 – Decentralization of the entire specialized care network and creation of the ONLINE REGULATION CENTER in SERGIPE;
2006/2007 – Development of SISREGIII at the MINISTRY OF HEALTH / DATASUS;
2011 – Development of the MANAGEMENT AND GOVERNANCE CENTER, supported by the INOVA-SE project of FINEP.
Objectives

SOAP (Subjective, Objective, Assessment, Plan) is the method used to record the patient’s progress. It allows for the concise and structured recording of subjective information provided by the patient, as well as objective impressions of their overall condition.
- S: Subjective (information provided by the patient);
- O: Objective (clinical observations and test results);
- A: Assessment (analysis explaining the meanings of subjective data);
- P: Plan (decision regarding the course of action to be taken).
